Q. Why can I not see the entire screen?

A. Check the resolution of your monitor, CMAGtracker is best viewed with a screen resolution of 1024 by 768 pixels.

Q. Is CMAGtracker HIPAA compliant?

A. Yes, as the Patient's Identifiable information is not collected on this website.

Q. If I do not use the Patients' ID how will I then be able to keep track of the patient?

A. Make sure that you assign Patients Ids that are not their Social Security Numbers or any other Hospital or Managed Care Id. To keep track of the patient you must write down the Id number you created for the patient on the patient records you keep. This way you will be identify the patient while keeping the data anonymous on the website. You may use the downloadable Patient Tracker.doc.

Q. Why do I need patient consent and how do I go about receiving it?

A. As the CMAGtracker is collecting data that will allow CMSA to validate the CMAG guidelines patient consent is required. Please refer to the Patient Privacy and Consent Section for details.

Q. How many assessments do I need to conduct?

A. For the purposes of the study we need a minimum of 3 assessments, however you may conduct as many as you deem necessary.

Q. I need help, what do I do?

A. Please email you specific questions to Administrator@cmagtracker.org and you should receive a response within 2 business days.

Q. What happens if the patient is not in my care after a few months?

A. Please close the patients file by completing the outcomes section and note accordingly. If the patient is transferred to another case manager within your group and he/she is taking part in CMAG validation then the next case manager should be able to see the patient via the company log-in (to be launched November 2004).
